Thank you for attending our conference. Please complete this evaluation. There are four sections: 1) speaker evaluations, 2) required questions, 3) overall event evaluation, and 4) CME / MOC credit claiming.
After you submit your evaluation, you will receive a confirmation screen with the link for your CME Certificate. Copy and paste into your browser to download our blank certificate. Then fill out your name and number of credits claimed for your records.
MOC Quiz
If you are requesting MOC points, we encourage you to first complete the evaluation and enter your CME credits claimed. Then complete this required MOC quiz from our speakers. A passing grade of 60% is required to receive MOC points.
We report learners claiming MOC monthly, please allow processing time for this to reflect.
